Getting Started on Your Fertility Journey

When it comes to fertility, no two journeys are alike. This is why our doctors take the time to meet with you and your partner (if applicable) to review your medical history and learn about your fertility goals. Whether your goals are trying to have a baby, or wanting to freeze your fertility for the right time, we can offer a solution.

Before Your Appointment

After making your first appointment with Tennessee Fertility Institute, you will receive an email link with a username and password for our patient portal and electronic medical records (EMR) system, Prelude Connect. We ask that you complete all of the necessary forms in the portal as soon as possible to secure your appointment. Click here to access Prelude Connect.

Much of the first appointment is spent reviewing past medical history. Thus, we ask you to complete the TFI medical release form  at least 3 days before your first appointment, so that we can obtain your medical records. You may also bring your medical records to your first appointment if you cannot access them beforehand.

What to Bring to Your First Appointment

On the day of your appointment, we ask that you arrive 15 minutes before your scheduled appointment time. Please complete and bring the following to your appointment:

  • Insurance card and insurance coverage information
  • Photo ID
  • Insurance co-payment or self-payment

What You Will Accomplish During the First Visit

As you meet with your provider, you and your partner (if applicable) will have the opportunity to ask any questions and address any issues.

It might be recommended that you and your partner have additional medical testing to help identify the cause of your difficulty getting pregnant. Particular care will be taken so that only the appropriate tests are conducted, saving both time and expense.

What Happens After Your First Visit

After you have completed your initial consultation, you will meet with our dedicated financial counselor to discuss costs, insurance coverage, and possible payment options tailored to your treatment plan. We work with every patient to find the best financial solution to complement their treatment plan.
